Your initial appointment will consist of a full, comprehensive evaluation of your mouth, including an Oral Cancer Screening Test. In addition to the clinical knowledge obtained, any necessary digital X-rays will be taken to aide in the diagnosis and ultimately a treatment that meets all of your Periodontal and Dental Implant needs. All treatment options will be reviewed in detail with you to develop an appropriate treatment plan.

If you are presenting with dental pain, we will make every effort to address this pain immediately, in an emergency manner, to make you comfortable that day.

To assist us in delivering outstanding care and to make your first visit with us as smooth as possible, please assist us by providing the following information at the time of your consultation:

    • Personal Identification
    • Your referral slip from your Dentist and any X-rays, if applicable
    • A list of medications you are presently taking

If you have dental insurance, please bring your current dental insurance card. If you don’t have one, we will need the subscriber name, subscriber number, group number, and insurance carrier’s phone number. This will save time and allow us to help you in the processing of your claims.

IMPORTANT: All patients under the age of 18 years must be accompanied by a parent or guardian at the consultation visit.

Please alert the office if you have a medical condition that may need special consideration prior to treatment (i.e. diabetes, high blood pressure, artificial heart valves and joints and/or drug allergies and pre-medication requirements).

X-Rays

If your dentist has taken X-rays, please have them forwarded to our office as they may be helpful in our diagnosis and treatment plan. Any digital X-rays can be emailed to us. If additional films are necessary, they can/will be taken at our facility.
